There really are only 2 binding requirements, everything else is up to you.
1. The author/s name must be displayed clearly on all production material and on all posters and programs.
2. An endorsement must be displayed on all posters and programs in a reasonably noticeable position. The endorsement must read “Produced with the permission of Stage-Write Plays, Australia (www.stage-write.com.au)”
